All,

I had on the agenda for the last meeting creation of a SC wiki page
(It was an action item from the meeting before that), but somehow it
got skipped.  Here is the link:

Linked on that page is ANOTHER action item I had, which was a request
form.  I created a google form for it, but that's all new to me, so
it's still considered a draft.

If anyone has input on the wording on the web page, or the structure
of the request page, please let me know.
